One was not able to insert a tube from an orthograde view for the severe stenosis of the D. choledochus in a pancreatic head carcinoma with typical double-duct-sign. A PTC with a followed passage of the stenosis with wire was done and the securing of the access per drainage performed. Subsequently, the patient was besieged and a GG-wire stent was placed endoscopically.
